Comprehensive Annual Physical Exam
Your health evaluation begins with a confidential medical history form that we send to you electronically before your appointment. You can complete the form at your convenience and submit to us before coming to our office or you can bring it with you for your visit. During your extended, 90-minute appointment, Dr. Chronos and his staff will review your family history, any current and past health issues, medications, immunization records and social/lifestyle factors.
An in-depth physical examination will be followed by an EKG, stress test and a full range of screening blood tests based on your needs. Our goal is to develop a thorough assessment of your current health status and to create a personalized, achievable healthcare plan. We also provide diagnosis and management of ongoing medical conditions as well as referrals to recommended specialists if they are needed.
Complete Cardiac and Vascular Risk Assessment
In the United States, the most common type of heart disease is coronary artery disease (CAD), that can lead to heart attack. Most heart disease is caused by natural aging. It can be seen, measured, and managed. We can better assess your heart health through non-invasive imaging. We can also use tests that can analyze the molecular size of the so-called ‘bad cholesterol’ and identify inflammation in the coronary arteries. Preventing heart disease is possible, by recognizing your symptoms and risk factors early.
Comprehensive assessment of all cardiovascular risk factors includes:

Lake Country Medical Concierge will provide you with the tools you need to live a heart healthy life. We will help to define and develop a health plan tailored specifically for you, with lifestyle recommendations and medication management as needed. While statins can be extremely successful at lowering the quantity of cholesterol, only lifestyle has proven to actually alter the quality of cholesterol. About half of all heart attacks occur in people with “normal” cholesterol levels. We recognize that ongoing education and support to promote a healthy lifestyle puts you in the driver’s seat towards optimal health.
Strokes can be actively prevented. High blood pressure (hypertension) is the single most important risk factor for stroke. Even mild hypertension, if untreated or treated poorly, increases stroke risk.
Other controllable risk factors for stroke relate to your heart health. It makes sense that keeping your heart and blood vessels as healthy as possible will reduce your stroke risk. The most common cause of a stroke is a blood clot. Atrial fibrillation, a common heart rhythm disorder, can cause blood to pool in the heart’s upper chambers and form blood clots. With our in-house advanced imaging capability, we can look for any evidence of irregular heart rhythms that might indicate afib.
Lifestyle issues such as weight management, sleep, and stress are all risk factors that can be assessed with recommendations that are included in your health plan for stroke prevention.
Cognition Function & Memory Loss Assessments
Many of us are concerned about the loss of cognitive function and memory as we age. In normal, healthy cognitive aging, people experience occasional memory lapses that don’t interrupt daily life, and they sometimes have difficulty recalling words or remembering names. Cognitive decline that extends beyond normal aging may be a sign of a memory disorder.
At Lake Country Medical Concierge, we take an in-depth approach to evaluating cognitive function. Blood tests can help rule out any physical problems that can affect memory, such as a vitamin B-12 deficiency or an underactive thyroid gland. We may also recommend an MRI or CT if advanced neurological imaging is needed.
We are currently evaluating the best tablet-based assessment tools that will allow us to establish a baseline and track our patients’ cognitive abilities year-over-year. Finally, through our highly qualified network of cognitive specialists, we can recommend more comprehensive neuropsychological testing based on need.
