Nicolas Chronos, MD, FACC, FESC
Dr. Nicolas Chronos is director of Lake Country Medical Concierge. He is an internist and interventional cardiologist well known for groundbreaking research in heart disease treatment. Dr. Chronos specializes in general cardiology, coronary artery disease management and heart failure management. He is also an expert in cardiac regenerative medicine, cardiovascular medical device assessment and translational cardiovascular medicine. Dr. Chronos partners with the University Cancer and Blood Center, headquartered in Athens, Georgia, to provide cardio-oncology treatment.
Previously, Dr. Chronos worked at the Atlanta Cardiology Group, where he developed and served as CEO of the Saint Joseph’s Translational Research Institute. Dr. Chronos has held academic appointments at Duke University and Stanford University as a consulting professor of medicine and cardiology working with researchers in preclinical and clinical trials.
Dr. Chronos received a Bachelor’s degree in Medicine and Surgery from the Royal Free Hospital School of Medicine in London in 1987. He completed specialized training in cardiology and subsequently interventional cardiology at the Royal Brompton National Heart and Lung Institute in the UK. In 1992, he was awarded a British Heart Foundation International Fellowship and moved to the United States to continue his research in thrombosis and interventional cardiology at Emory University School of Medicine. He was named Director of Research at the Andreas Gruentzig Cardiovascular Center at Emory University Hospital in 1997.
Dr. Chronos is board certified by the Royal College of Physicians (London) in internal medicine and cardiology (FRCP). He is affiliated with Massachusetts General Hospital’s Vaccine and Immunotherapy Center and is a scientific advisor for the International Society for Cardiovascular Translational Research. A Fellow of the American College of Cardiology, the European Society of Cardiology and the Royal College of Physicians of London, he has published several books, more than 200 peer-reviewed articles, and authored a series of patents in the medical device field. As a speaker at major conferences and meetings both nationally and internationally, Dr. Chronos is highly sought after to share his intellectual and practical contributions in the field of cardiovascular medicine and research.

Kimberly Warnock, NP-C
Kim is a Family Nurse Practitioner who brings over twelve years of experience providing exceptional care for both acute and chronic illnesses. After years of working in the Emergency Department, Kim has seen the true value of comprehensive preventative medicine, diabetes care, and management of chronic illness and it has become her passion. Kim is a highly valued addition to our team.
Kim graduated with her Bachelor’s degree from Georgia College and State University in 2009. She then practiced as a registered nurse in inpatient cardiology and Emergency Medicine. Kim became a Nurse Practitioner in 2013, graduating with a Master of Science in Nursing from Medical College of Georgia. Kim is board certified by the American Association of Nurse Practitioners.
For the past nine years Kim has practiced Emergency Medicine in multiple Emergency Departments in the area, treating patients of all ages, complaints, and acuities. This experience makes Kim exceptionally well rounded and capable of providing our patients with the absolute best healthcare. Kim and her husband have lived in the Lake Oconee area since 2009 and have three wonderful children. She enjoys time on the lake and traveling. She looks forward to taking care of patients and building relationships in the community she lives in.
RJ Brooks
Vice President of Operations
RJ Brooks directs the day-to-day functions of Lake Country Medical Group. RJ coordinates with our care team to provide all our patients with a smooth, hassle-free experience. RJ has over 10 years of experience in durable medical equipment. His passion is facilitating an environment where patients can enjoy excellent health care with convenience and comfort.

Heather Scott
Concierge Clinical Manager
Heather Scott is a Certified Medical Assistant with more than 10 years of experience in healthcare. She serves as the Concierge Clinical Manager for Lake Country Medical Concierge, where she oversees clinical operations and helps ensure members receive the exceptional service and attention they have come to expect.
With years of experience supporting providers, patients, and clinical teams, Heather plays a vital role in keeping the Concierge program running smoothly. She works behind the scenes to coordinate workflows, support staff, and help maintain the level of responsiveness and efficiency that allows our team to focus on what matters most: our patients.
Known for her leadership, dependability, and problem-solving skills, Heather is a trusted resource for both patients and team members. Her commitment to excellence and passion for healthcare help create a positive experience for everyone who walks through our doors.
Originally from Jones County, Heather brings a strong sense of community and dedication to service in everything she does. Outside of work, she enjoys spending quality time with her family.
Kinsley Payton
Lead Medical Assistant
Kinsley is an integral part of the Concierge care team, helping ensure each patient receives attentive, personalized service from the moment they arrive. She works closely with our providers to prepare for visits, coordinate patient care, and assist with clinical needs. Known for her friendly demeanor and commitment to excellence, Kinsley helps create the smooth, high-touch healthcare experience that sets Concierge Medicine apart.
Jenna Grundner
Licensed Practical Nurse
With nearly a decade of nursing experience, Jenna brings compassion, energy, and a diverse clinical background to every patient interaction. After earning her diploma in Practical Nursing from Central Georgia Technical College, she spent years caring for patients in mental health, addiction medicine, and pediatrics before transitioning into remote concierge care.
Today, Jenna works closely with patients to monitor their health needs, provide personalized support, and help guide them toward living healthier, more balanced lives. Her approachable nature and genuine passion for helping others make her a trusted and reassuring presence throughout each patient’s healthcare journey.
She believes healthcare should feel personal, approachable, and empowering. Jenna is the kind of nurse who listens first, helps patients feel heard, and works alongside them to find thoughtful, practical solutions while always keeping the human side of care at the center.
Outside of work, Jenna lives in Woodstock with her partner, her two sports-loving kids, Audrey and Lukas, and what feels like an ever-growing collection of craft supplies. When she’s off the clock, you’ll usually find her painting, starting a new DIY project she may or may not finish, or spending time outdoors with her family.
